    We are currently seeking a Weekend Visit Nurse responsible for the assessment, coordination and delivery of care to Suburban Home Health patients. The Weekend Visit Nurse supports patients in the counties of Hamilton, Tipton, Boone, Clinton and Howard.


    The Weekend Visit Nurse is an hourly position providing direct patient care in the home to adult and geriatric patients.
    - The Weekend Visit Nurse completes routine visits in the home setting as directed by the physician ordered plan of care
    -Collaborates with physicians and makes recommendations regarding alternative treatment, plan of care and agency resources
    -Facilitates communication with interdisciplinary team regarding patient goals and coordination of care
    -Assures accuracy and completeness of clinical documentation and communicates with physicians and clinicians to ensure outcomes are met
    -Performs duties and responsibilities in a manner that adheres to all agency policies and procedures
    -Provides education to patients and caregivers regarding medical condition, treatment plan and coordination of care
    -Responds to phone calls within 30 minutes to ensure patient needs are met
    -Attends monthly nursing meetings, education sessions, and quarterly all-staff meetings


    -Degree in nursing from an accredited college/university required. LPN or RN.
    -Must possess current license from the Indiana State Board of Nursing.
    -At least two years' experience in nursing required.
    -Must possess exceptional verbal and written communication skills.
    -Ability to prioritize work, utilizing fundamental time management skills.
    -Must possess valid Driver’s License and automobile insurance.
