The RN Case Manager is responsible for the assessment, coordination and delivery of care to Suburban Home Health patients.
- Collaborates with physicians and makes recommendations regarding alternative treatment, plan of care and agency resources
- Facilitates communication with interdisciplinary team regarding patient goals and coordination of care
- Assures accuracy and completeness of clinical documentation and communicates with physicians and clinicians to ensure outcomes are met
- Develops and implements goal-directed plans of care to ensure patient achieves desire outcomes
- Completes accurate OASIS documentation within agency expectations
- Performs duties and responsibilities in a manner that adheres to all agency policies and procedures
- Provides education to patients and caregivers regarding medical condition, treatment plan and coordination of care
- Responds to phone calls within 30 minutes to ensure patient needs are met
- Completes an on-call log regarding patient condition and resolution provided
- Attends monthly nursing meetings, OASIS education sessions and quarterly all-staff meetings
- The RN Case Manager provides on-call coverage up to twice a week beginning at 4:30 pm and ending at 8:00 am the following morning. On-call coverage can include the following tasks: troubleshooting wound vac’s and infusion devices, catheter and ostomy care and triaging all phone calls regarding patient concerns.


- Degree in nursing from an accredited college/university required. BSN preferred.
- Must possess current license from the Indiana State Board of Nursing.
- At least one year’s experience in nursing required.
- Previous experience and/or knowledge in home health preferred.
- Must possess exceptional verbal and written communication skills.
- Ability to prioritize work, utilizing fundamental time management skills.
- Must possess valid Driver’s License and automobile insurance.
