We are searching for a Home Health Aide, Certified Nursing Assistant to serve the Hamilton County and surrounding areas.

Responsibilities

An associate in this position provides direct patient care for adult and geriatric patients in the home setting. Provides and assists patients with personal care and activities of daily living in their place of residence. Performs duties as directed by the supervising Registered Nurse (RN). Associate is capable of delivering care appropriate to patients. Communicates changes in patient status to supervising RN. Creates and submits legible, accurate and timely documentation. Shows sound judgment in care being provided. Adapts care to individual patient needs including age-specific and cultural needs. Supports patient rehabilitation, encourages client independence, involves patient in care plan and respects patient's rights. Performs duties in compliance with regulations, policies and procedures. Demonstrates attention to details for patient's safety and safety of their home environment. Reports unsafe conditions in the home. Practices effective infection control and adheres to infection control policies and procedures. Demonstrates safe driving record. Practices safe lifting and transferring of patients. Demonstrates effective body mechanics during provision of care. Adapts to change in patient assignments. Makes recommendations for improving patient care and services. Represents the organization as a reflection of its mission. Communicates clearly and positively with patients, families, team members and others. Displays caring behavior. Keeps patients and family informed regarding visit time and schedule changes.

Qualifications

Must have current Home Health Aide registrar AND CNA.
Must be able to provide own transportation to assignments.
Ability to work independently.
Ability to work in and support a team environment.
